Optro Research Highlights AI Governance Challenges in Enterprises

Optro, an AI-driven GRC intelligence platform, has uncovered significant challenges in enterprise AI deployment. According to their 2026 report, AI agents are increasingly executing high-stakes decisions without adequate organizational oversight, leading to errors. Notably, 34% of surveyed organizations reported acting on incorrect AI agent decisions.

The survey, involving over 400 leaders in governance, risk, and compliance, reveals that while most agree on the need to adapt workflows for AI, only a small fraction have done so safely. This mismatch between AI capability and oversight causes increased risks.

AI agents are being entrusted with decisions like approving transactions and modifying policies. However, failures in governance are common, with 30% witnessing unintended AI actions and 25% encountering control failures. Moreover, 46% of employees now spend more time addressing AI outputs than before, highlighting the governance gap.
